Create a Linear issue from a conversation

/api/v1/accounts/:account_id/integrations/linear/create_issue

Opens an issue in Linear, links it to the conversation, and writes an activity message into the thread so the history shows who raised it.

Available to anyone who handles conversations — unlike disconnecting, which is admin-only.

Every call here goes out to Linear. When Linear refuses, the reason comes back as a 422 with its message rather than being swallowed.
