Upload a file

/api/v1/accounts/:account_id/upload

Stores a file and answers with its URL and a blob_id to attach elsewhere. Two ways in, one of which is required:

• attachment as multipart, for a file you hold.
• external_url, to have the server fetch it. The fetch is guarded — it refuses addresses that resolve inside the network, files past the size limit, and content types that are not allowed — so each of those comes back as its own 422 rather than a generic failure.
