Add a document to an assistant

POST 
/api/v1/accounts/:account_id/assistant/documents

Either a link to crawl or a PDF to upload — send multipart/form-data for the latter.

Adding one is what starts the extraction of question-and-answer pairs from it; those arrive as pending responses.

The account's plan caps how many documents an assistant may hold. Going over it answers 422 saying so, rather than accepting the document and failing later.
